You are the new HIM Director in a small rural hospital. One of your responsibilities is CDM maintenance. You are surprised to learn that there is no formal process in place for chargemaster maintenance. Requests for additions are supplied by individual departments directly to IT for entry into the chargemaster. You decide to design an improved process for chargemaster maintenance.
Make a form or describe the workflow to be used when new CPT codes are introduced to ensure that all data elements required in the CDM are identified, verified, and signed off on by you and the appropriate departments for inclusion in the CDM.
Assume the codes identified below are five new/revised CPT codes for the upcoming calendar year. These will need to be included within the facility’s CDM.
For these new codes, identify which department and or medical service(s) will need to be consulted to complete your new form. (hint: look at the section/category these codes are located in within the CPT book to assist you) Identify who from that department will provide the data and how/when it will be obtained.

94640 Pressurized or nonpressurized inhalation treatment for acute airway obstruction for therapeutic purposes and/or for diagnostic purposes such as sputum induction with an aerosol generator, nebulizer, metered dose inhaler or intermittent positive pressure breathing (IPPB) device
